  1. Search by name or CPSO number.
  2. Confirm the practice location and registration status.
  3. Review public notices or restrictions shown on the profile.
  4. Call the clinic to confirm patient acceptance.

Keep your information organized

Write down clinic names, dates contacted and the next step each office gave you. Keep medical information with your care providers. Do not send diagnoses, health-card numbers or records to Aafiyat Outreach Centre.
